- a summary of the post-incident investigation and its root-cause detemination.
-your opinion no where in the life-cycle the root cause could have been found, and how
-your opinion as to what could have been done in the product architecture to prevent this disaster.
-if this was not a software disaster, how do this incident and the lessons learned relate to software design
- why did you select this particular disaster? what personal lessons have you learned(i.e., how have you changed as an engineer after studying this disaster)
