Case Study:

Bruce Eggert, age 59 years, was brought to the ED with complaints of dizziness, dyspnea, restlessness, and anxiety.  Mr. Eggert currently works as an accountant for a large firm.  He is married and has two children living at home.  He reported a 2 day history of hematemesis with some bright red blood and large amounts of “coffee ground” emesis.  Mr. Eggert denied any recent or chronic illnesses and was unable to remember if anyone in his family had ever had problems with GI tract bleeding.  He did admit to drinking six to eight alcoholic beverages almost every day for the past 7 years.  Initial assessment revealed cool and clammy skin, a distended abdomen with hyperactive bowel sounds, and tachycardia. He is allergic to Codiene, Full Code, Ht: 59” WT 210lbs. Current vital signs and laboratory results are as follows:

BP – 92/60 mmHg HR – 120 bpm Respirations – 28 breaths/min

Temp – 36.9° C (98.3 °F oral) Ammonia 60 mcg/dl

Glucose – 87 mg/dl LDH – 500 U/L AST (SGOT) – 950 U/L

ALT (SGPT) – 1000 U/L Alkaline Phosphatase – 165 U/L

Total Bilirubin – 2.5 mg/dl Albumin – 2.3 g/dl

PT – 26 sec PTT – 85 sec

Three hours after arriving in the ED, Mr. Eggert was admitted to the ICU with an IV of NS.  Two units of PRBC’s were administered.  Twenty units of vasopressin (Pitresssin) in 100 ml of D5W were given IV over 20 minutes.  A continuous infusion of vasopressin at 0.4 units/min was then initiated.  Sublingual NTG was added to the medication regimen.  Diagnostic fiberoptic endoscopy, immediately preceded by a saline lavage, was scheduled for the following day.  Endoscopy revealed a large esophageal varix (1.5 cm) above the gastroesophageal junction.  Only a small amount of bright red blood was observed, so sclerotherapy was performed.  A solution of 5% ethanolamine oleate was given by intravariceal injection.  Mr. Eggert’s condition remained stable after sclerotherapy and he was transferred to a medical floor.  Subsequently sclerotherapy sessions were scheduled on a weekly basis for 4 weeks.

In this paper I discuss Richard Rodriguez’s perspective of the American education system. Rodriguez had a complicated relationship with formal education. On one hand he was very appreciative of the great teachers and the academic opportunities that were made available to him, but on the other hand he feels that success in the education system came at the expense of his individuality.

 

In recent years, there has been a great deal of debate over the definition of intelligence. We have coined terms such as “book smarts” and “street smarts” to categorize different types of intellect. In his youth, Richard Rodriguez had not yet been introduced to the concept of different types of intelligence; the only notion of intelligence that he had was “book smarts”. This misunderstanding of intelligence informed the academic methodology he employed, namely, that of a mimic. Likewise, it informed his opinion of his parents as simpleminded. As Rodriguez evolved as a person, so too did his perception of intelligence. This expanded perspective imbued him with newfound appreciation for his parents’ intelligence. Ultimately, it is through this personal growth that Rodriguez is able to rekindle his relationship with his family and reclaim his cultural identity.

 

Rodriguez discovers a parallel between his own life and that of the archetypical academic described by author Richard Hoggart in the chapter “Scholarship Boy” from his book Uses of Literacy. According to Hoggart, the scholarship boy over-evaluates the importance of examinations, of the acquisition of knowledge, and of hoarding the opinions of others. The perception of the scholarship boy as intellectual is only illusory, because the scholarship boy mistakenly concerns himself with acquisition of facts rather than of the handling and use of facts. The self-destructive cynical lens through which the scholarship boy views education persists into his adulthood; he begins to perceive life as a series of examinations where he must trounce the competition. All these characteristics that Hoggart describes as intrinsic to the scholarship boy are very much present in Rodriguez throughout his youth. Although Rodriguez achieved a large degree of success in his academics, he did not truly learn much at all due to his passive approach towards education. While Hoggart chastises the scholarship boy for his passive approach to education, Rodriguez argues that in this respect the scholarship boy is but a victim of circumstance. That the scholarship boy does not spend hours mindlessly accumulating factoids by his own volition; he does it because it is what the education system expects from him. If he ignored these expectations, fulfilling his academic ambitions would be impossible. Ask not what your education can do for you - ask what you can do for your education.

 

            Just as Rodriguez cannot be faulted for his passive approach to education, he must also not be faulted for his disparaging opinion of his parents' intelligence.

Section I/Establishing World Trade: The Geography and Philosophies of Early Economic Systems, 1300-1500

1. What are the basic requirements for the functioning of a free market economy? How did the Great Depression of the 1930s affect thinking about the operations of free markets?

2. First describe the basic components of a free market economy. Then describe the historical criticisms of free markets developed by Karl Polanyi and associates, and the counter-arguments developed by Philip Curtin.

3. According to the author of the textbook, “All societies--- regulate trade to some degree in order also to serve the non-economic goals of the society. Business may be more or less regulated, but it is never completely unregulated.” Cite at least three examples of such regulation from the chapter. In each case, who was actually regulating trade and how? What “non-economic goals” were being served? Based on evidence, examples and arguments from this chapter, how much should a society’s economy be regulated, by whom and for what reasons or ends, if any?

4. Discuss why, of all the general phenomena that have affected the way groups of people have lived, world trade is important for understanding relationships in a “world system” and how and why people lived as they did in a particular place and time. Use Emmanuel Wallerstein’s “world system” theory to articulate your response.

5. Explain, by making use of the story of the growth of the world system of the post-Roman world to 1450, what it means to say, as historian Janet Abu-Lughod argues, that “world systems” evolve and sometimes restructure.

6. Discuss the nature and importance of trade in the Americas in the 1400s.

7. Write an essay on West Africa and East Africa trade in the 1400s that includes discussion of main routes, how goods were transported, and what the principal goods were.

8. From approximately 750 to 1500 C.E., central Asian, trans-Saharan and Indian Ocean trade routes were dominated by Muslim traders, mostly Arabs, Berbers and Persians. What are the most likely reasons for this Muslim dominance? What were some of its principal effects? Cite specific from the textbook.

9. Between 1407 and 1433, the Ming Emperor of China sent his great Admiral Zheng He on seven great seaborne expeditions, extending as far as India, the Persian Gulf and east Africa. But the voyages were discontinued and never followed up by the extension of Chinese political or commercial control to those areas, in spite of the fact that China was probably the richest and most powerful country in the world. Why was this the case? And what, according to many historians, were some of the consequences of this withdrawal from international engagement by the Ming dynasty?

Section II/Western Europe in a Restructuring World System, 1300-1500

10. Describe the political and economic characteristics of feudalism.

11. Cities gained in political and economic influence in the 1300s and 1400s in western Europe. Describe the various forces that led to this phenomenon, and then discuss the class structure of the typical cities and the various social and economic conflicts within the cities.

12. Chapter 13 discusses no fewer than five important social, political and cultural consequences of the flowering of the European economy from the 11th through the 15th centuries C.E. List as many of these consequences as you can and then review at least two of them in detail, explaining specifically how economic growth brought about the political, social or cultural change under discussion.

13. According to the textbook, Jews had become “so much a part of the merchant classes in early medieval northern Europe that a traditional administrative phrase referred to ‘Jews and other merchants’.” Why was this the case? In what respects were Jews the classical “stranger merchants” referred earlier in chapter 13? What were the effects of this on the status of Jews in Christian Europe?

14. In the extract from the Summa Thiologiae by St. Thomas Aquinas (p.401, 3rd edition of the textbook), the text cites evidence of a major revision of the policies of the Christian Church regarding business and trade. What had been the earlier view of the Church? How does Aquinas reflect a rethinking of the Christian position? What may have been some of the reasons for this change? What were some of the consequences for the Church, for traders and businessmen, and for Jews in Europe?

15. The Renaissance was a major period of growth in European history. Describe the most important intellectual, cultural, and scientific achievements of this period.

16. What was the important advice of realpolitik that NiccoloMaciavelli, author of the famous handbook of political philosophy titled The Prince (p.438, 3rd edition of the textbook), offered to rulers?

Section III/The Unification of World Trade: New Philosophies for New Trade Patterns, 1500-1776

17. Discuss the Spanish conquest of the Aztec and Inca civilizations of Mexico and Peru, with particular reference to the following: How were a mere handful of Spanish conquistadors able to subdue these two powerful empires in so brief a period of time?

18. Describe the different types of economic organization used by the Spaniards in the Americas to exploit the peoples of the region for Spain’s- and Europe’s- profit in the succeeding years since conquest.

19. Based on what you have learned from chapters 12 and 13, what were the principal causes of the Protestant Reformation? What were the major objections raised by Luther, Calvin and other reformers against Catholic doctrines and practices? Why were the reformers able to gain such widespread acceptance in some areas of Europe but not in others? How might economic and technological developments have contributed to the Reformation?

20. In what way, according to Max Weber, did Calvinistic Protestantism promote the rise of modern capitalism in Europe? What particular developments in Holland, England, northern Germany, and the United States convinced him to believe that there is a link between capitalist growth and Protestantism? Also, present the counter argument by the British social historian R.H. Tawney.

21. In some ways the decline of the Spanish monarchy parallels the classic model of imperial decay, as outlined in Chapter 5 (Volume I: to 1500). Using the model presented in that chapter and specific evidence and examples from Chapter 13, explain why the Spanish Empire began to decline, c.1570-1700. [If you do not have Vol.I of the Spodek book, use the information about Spain’s decline in this chapter and try to formulate your own general model]

22. Compare the reasons for the rise of France and the Dutch Republic as major trading powers in the 17th century. What advantages did they enjoy that Spain and Portugal did not?

23. How does the author of the textbook account for the success of Great Britain in establishing the largest and most enduring economic empire of all the European powers during the eighteenth and nineteenth centuries?

24. Why was a free market economy the best system of exchange, according to Adam Smith? Why was laissez-faire preferable to regulation by church or state? How would individual or corporate greed be controlled under such a system, according to Smith? Section IV/Migration: Demographic Changes in a New Global World, 1300-1750

As a healthcare professional, you may be called upon to provide expertise, guidance and/or opinions on healthcare matters as they are debated for inclusion into new policy. You may also be involved in planning new organizational policy and responses to changes in legislation. For all of these reasons you should be prepared to speak to national healthcare issues making the news.


In this Assignment, you will analyze recent presidential healthcare agendas. You also will prepare a fact sheet to communicate the importance of a healthcare issue and the impact on this issue of recent or proposed policy.


To Prepare:


Review the agenda priorities of the current/sitting U.S. president and the two previous presidential administrations.

Select an issue related to healthcare that was addressed by each of the last three U.S. presidential administrations.

The term “knowledge worker” was first coined by management consultant and author Peter Drucker in his book, The Landmarks of Tomorrow (1959). Nurses are very much knowledge workers. What has changed since Drucker’s time are the ways that knowledge can be acquired. For this assignment, select one of the following options:

Option 1: Imperialism

The exploitation of colonial resources and indigenous labor was one of the key elements in the success of imperialism. Such exploitation was a result of the prevalent ethnocentrism of the time and was justified by the unscientific concept of social Darwinism, which praised the characteristics of white Europeans and inaccurately ascribed negative characteristics to indigenous peoples. A famous poem of the time by Rudyard Kipling, "White Man's Burden," called on imperial powers, and particularly the U.S., at whom the poem was directed, to take up the mission of civilizing these "savage" peoples.

After reading the poem, address the following in a case study analysis:

• Select a specific part of the world (a country), and examine imperialism in that country. What was the relationship between the invading country and the native people? You can select from these examples or choose your own:

o Belgium & Africa

o Britain & India

o Germany & Africa

o France & Africa

• Apply social Darwinism to this specific case.

• Analyze the motivations of the invading country?

• How did ethnocentrism manifest in their interactions?

• How does Kipling's poem apply to your specific example? You can quote lines for comparison.

Option 2: The Armenian Genocide

Ethnic hatred is not new to the human race and ethnic hatred also goes hand in hand with some of the causes of World War 1 that we have discussed this week, namely nationalism and imperialism. In nationalism, the people not only believe their countrymen (those who look, talk, sound, and believe like them) are not only "better" than people of certain other countries but they also have the right and possibly even the responsibility to conquer areas with those inferior to them and take whatever resources they feel they need. Add to this the need for the leader of a country to be able to blame a certain ethnic group within that country for their problems. Such was the case in the Ottoman Empire. The Ottoman Turks launched a winter offensive into the mountains of southern Russia early in the war and suffered a severe defeat, both at the hands of the Russian army and the terrible weather they encountered. The Sultan needed a scapegoat to blame the loss on, so he picked an unpopular ethnic minority, the Armenians. Since the people seemed to believe the story, the Ottoman military rounded up as many Armenians as they could and marched them away to their fate.

After viewing the videos, conduct research on the topic and develop a case study based on the following questions:

• How did the Armenian genocide play on the root causes of World War I, including nationalism, imperialism, social Darwinism, ethnic hatred, and the gullibility of the Turkish people who allowed this to happen and often even supported it.

• What were the main motivations of the Ottoman rulers who decided to try and eliminate an entire ethnic minority within their country?

• What was the process the Ottomans went through to round up the Armenians and march them to their deaths? Describe the process of the march and eventual genocide of the Armenians.

• Why do terrible crimes against humanity always seem to happen during a war?

• How do foreign governments recognize and deal with this event in their relationship with Turkey?

• Length: 2-3 pages (not including title page or references page)

• 1-inch margins

• Double spaced

• 12-point Times New Roman font

• Title page
